Forms All potential volunteers for Take Flight Farms must undergo a check with the State of Nebraska Child/Adult Abuse and Neglect Registries. This ensures the utmost safety for our program participants. OR Minor Volunteer Application Packet NOTE: These pdf files each contain six separate forms that must be completed and MAILED to Take Flight Farms.
